If you decide to use insurance to pay for services the first step is to check with your Managed Care Customer Service Representative. Managed Care services may or may not require prior authorization for services. Please ask the customer service representative of your insurance company if authorization is required and how to obtain this authorization. Ask the representative if you have a deductible, whether you have met this deductible, your co-pay per session, and the limit on how many sessions you are offered per year. If you haven’t met the deductible it is required that you pay the insurance contract rate per session until the deductible is covered.
Download and complete the Client Information Form and Client Consent to Treatment & Letter of Agreement (see the Forms page) and bring with you to the first session, or arrive 10 minutes early to complete these forms. In addition, please bring your insurance ID card and co-pay to the first session.
RATE
Private Pay Rate for individuals, couples and families: $150 per session.
